Program Support Officer, Integrated Care

- Starting salary $62K + superannuation + eligibility for salary packaging

The Program Support Officer will be required to provide administrative support for the Integrated Care leadership team at a critical and exciting time, contributing to NWMPHN's response to meet the identified needs of vulnerable communities across the north west Melbourne region.
The Integrated Care team's focus is on commissioning of projects, programs and services to facilitate, implement and scale integrated person-centred models of care across health and care settings. This includes primary, community and acute care, with a focus on reducing avoidable hospitalisations and improving shared care and communication across these care settings.
A typical week may include:
- Performing general administrative and program governance support, including developing resources and program documents, arranging meetings, coordinating papers, organising venues, minute-taking and following-up meeting actions
- Implementing systems and processes to ensure consistency, reliability and assurance of key functions and in line with agreed business processes
- Building and maintaining professional relationships with relevant internal and external stakeholders
- Developing resources, program documents and support communications related to Integrated Care projects/activity
We are looking for someone who has:
- Well-developed administrative and organisational skills and capabilities including demonstrated ability and competence in the use of technology and a variety of software systems
- Maintaining excellent standards of service in a high-pressure work environment requiring confidentiality, tact, judgement, and discretion
- Demonstrated ability to organise and maintain accurate records and up to date filing systems, and establish processes to ensure efficient and productive workflows
- Strong problem-solving skills, including identification and implementation of workable solutions to problems
